Speak to someone with a bit more knowledge than me (!!) but I'm thinking it's the diaphragm fingers gone.
Does the pedal feel king of springy?
If so I'm sticking with clutch plate diaphragm fingers.
It's worth giving it a bleed first just to make sure it hasn't got an air bubble in the fluid for some reason. Is the reservoir full or empty now?

The majority of hydralic clutch slave cylinders don't have any adjustment.
The major adjustments for a hydralic setup is the pivot ball inside the bellhousing or as you've played with, the pedal adjustment. The pedal adjustment is just for setting clearance of rod to master, not making adjustments to the amount of travel or anything.
It's possible that this was maybe slightly over adjusted and there has been slight pressure being applied to the clutch the whole time. This would lessen the clamping load.
Is the pedal firm immediately right from the top?

Q5 has got a bit small now. Need something a bit bigger.
I still like the carbon spoiler on the GT-R but will leave it off for a bit.
