My guess is that it's failing because the bolt wasn't properly tightened: Too loose and it could back out by vibration. Too tight, and it could break.

Borrow or rent a torque wrench. Or buy an inexpensive pointer-type wrench.

Clymer's give the specs as:
A12 109-116 ft-lb (15-16 Mkg)
A13, A14 109-145 ft-lb (15-20 Mkg)
