Top: 4-speed
Bottom: 5-speed
Note the 56A markings. 4-speeds can have the mark in either position.
The better way to tell is the shape of the extension housing. As you can see by the yellow lines, the 5-speed has a rectangular blocky part to the extension. The 4-speed extension 'bulge' is nearly circular.

The easiest way to tell if it's a 5 speed is the position of the reversing light switch. the 4 speed has it out the right hand side & the 5 speed has it on top of the rear housing pointing backwards. Or try & put the box into 1st gear & see which way the output shaft turns in comparison to the input shaft. Also, the cast 56A appears to be in different places on the front housing, though this might also change around in the 4 speeds.

I've got each of these. Front housing is the same for both.
5-speed
-Sandwich plate differs - oil drain plug screws into this at the bottom.
-Tail Housing - bulkier to encase 1st gear. As mentioned above the reverse switch position differs from 4 speed.

4-speed
-Tail housing - oil drain plug screws into back.

No actually - I have a 4 speed in my shed which shifts into a 5th position but there's nothing there...

that's just slop in the shifter internals, there was no 'conventional' pattern 5 speed offered in the 56a size box, to my knowledge. The shift pattern was used because the gearbox was mainly there for a racing homologation, & 1st gear is only supposed to be used once, from the standing start. this puts all the other gears in next to each other & saves a bit of time changing gears.
