It will also fit H150 as H145/H150 housing ends are the same.
Overview

Use a Brake Proportioning valve to set the balance. These are large rear brakes.
R31 Caliper bracket modified to fit H145
Holes re-drilled for H145 housing pattern
Remove the axles. Cut the axle bearing retainers and remove the axle bearings. Now you can remove the drum backing plate, and but more importantly will be able to fit the adapter over the axles. New bearings can then be fitted.
Lay H145 drum brake backing plate over Adapter, mark the holes to drill
Finned R31 caliper bolts to the adapter
The factory adapter is thin, but adequately strong
Consider use appropriate shims inside of the caliper bracket to prevent the bearing from moving. The stock H145 setup using shims between backing plate and housing.
Handbrake Considerations
Fit the calipers facing forward so you can run the cables back and loop around and utilise the existing hand brake bracket and cabling on the diff.
You can use one of the R31 cables to do the RH side but the LH side is not the correct length. Have a brake shop make a new cable or mix-match at the wreckers

GM calipers fitted (for a handbrake-less setup). Commodore VK rear calipers are same as Australia-assembled Skyline R31 calipers -- but without the external fins of the R31 caliper which limit rim selection.
